Leaking 2 inch Ball Valve in Midstream Pipeline

We investigated the cause of leaking/failure in a 2″ diameter floating ball valve in a midstream pipeline. Graphite packing deformation appeared consistent with loads that would be generated from volumetric expansion due to water freezing inside the valve.
